Four Jobs Inside a 10-in-1 Oral Hair Formula

Malaysian shoppers looking at an oral hair supplement are right to ask what the capsule is supposed to do. Folli-Activ is a 10-in-1 oral formula. The useful way to read that label is as four jobs: the dihydrotestosterone (DHT) pathway, inflammatory signals at the follicle, the keratin shaft, and absorption of ingredients that otherwise pass through the body in tiny amounts. Each job has its own published literature. None of that literature used the Folli-Activ capsule as the test product.

DHT and the patterned scalp

Androgenetic alopecia is the common patterned thinning of scalp hair. In the 2014 trial report by Cho and colleagues, the authors state the standard biology directly: genetic factors and androgens underlie the condition, follicles miniaturize, the growing phase (anagen) shortens, the resting phase (telogen) lengthens, and DHT is a major player in that process.

The human genetic record agrees that DHT matters for scalp hair pattern. Peterson, Imperato-McGinley, and colleagues described males with steroid 5-alpha-reductase deficiency. These adults had higher testosterone, lower DHT, and testosterone-to-DHT ratios of 35 to 84, against 8 to 16 in unaffected men. They also had decreased body hair, a scant beard, a small prostate, and no temporal hairline recession. The enzyme that makes DHT is part of how a male hairline and a male prostate develop.

Saw palmetto, pumpkin seed oil, and nettle root sit in Folli-Activ because published work on those botanicals addresses 5-alpha-reductase activity, testosterone-driven prostate growth, or sex hormone-binding globulin. Those papers are covered in the ingredient essays on this site. A formula aimed at patterned thinning has a reason to include more than one androgen-pathway ingredient, because the published actions are not identical.

Inflammation at the follicle

Hair is not only an androgen target. Philpott, Sanders, Bowen, and Kealey cultured human hair follicles and tested immune cytokines. Interleukin-1 alpha, interleukin-1 beta, and tumour necrosis factor-alpha were potent inhibitors of follicle growth. Inhibitory doses produced dystrophic anagen follicles: a condensed and distorted dermal papilla, vacuolation of the matrix, abnormal keratinization of the bulb and inner root sheath, disrupted follicular melanocytes, and melanin granules inside the dermal papilla. Inflammatory cytokines are capable of stopping a human follicle in culture and of distorting the keratin it makes.

Curcumin, green-tea EGCG, and aloe vera extract enter the formula from that direction. Curcumin has a published anti-inflammatory record acting through transcription factors and inflammatory cytokines. EGCG has a published record on human dermal papilla cells and on 5-alpha-reductase in cell-free enzyme assays. Aloe gel extracts reduced carrageenan edema and prostaglandin E2 production in the experiments of Vázquez and colleagues. Those are three different datasets. They are not one blended clinical trial.

Keratin is the shaft

The hair shaft is a keratin structure. Schweizer, Langbein, Rogers, and Winter catalogued the human keratin family: 54 members, of which 11 type I and 6 type II keratins are hair keratins. Nine further epithelial keratins are expressed specifically in the follicle, bringing the hair-follicle-specific set to 26. Mutations in a subset of the type II hair keratins cause inherited shaft disorders, including monilethrix. The shaft is built from named proteins, and those proteins fail in specific ways when the genes fail.

Biotin belongs with that job because of enzyme chemistry, not because a Folli-Activ trial exists. Zempleni, Hassan, and Wijeratne describe biotin as the coenzyme for five carboxylases: acetyl-CoA carboxylase alpha, acetyl-CoA carboxylase beta, propionyl-CoA carboxylase, 3-methylcrotonyl-CoA carboxylase, and pyruvate carboxylase. Those enzymes fix bicarbonate in pathways of fatty acid synthesis, amino acid metabolism, and glucose metabolism. The same review lists hair loss among the symptoms of biotin deficiency. Royal jelly sits beside biotin as a complex food secretion whose fatty acid 10-hydroxy-2-decenoic acid has a separate skin-barrier literature.

Absorption is a formula decision

An ingredient that never reaches the blood in a measurable amount does not perform its job. Anand, Kunnumakkara, Newman, and Aggarwal reviewed curcumin pharmacology: phase I trials found curcumin tolerable at 12 grams a day, and plasma levels stayed low because of poor absorption, rapid metabolism, and rapid elimination. Shoba and colleagues measured piperine, an inhibitor of hepatic and intestinal glucuronidation. In healthy volunteers, 2 grams of curcumin alone produced serum levels that were undetectable or very low. The same 2 gram dose taken with 20 milligrams of piperine raised bioavailability by 2000 percent in that study. In rats, 2 grams per kilogram of curcumin plus 20 milligrams per kilogram of piperine raised bioavailability by 154 percent.

Those doses are the doses in the pharmacokinetic study. They are not a statement of how many milligrams of curcumin or piperine are in a Folli-Activ capsule. The formulation fact is simpler: the bottle pairs curcumin from turmeric with piperine from black pepper, which is the pairing Shoba tested.
